Membership to a DPC clinic is not health insurance coverage nor does it replace health insurance coverage. This agreement does not provide comprehensive health insurance coverage. It only provides the health care services specifically described. A DPC membership does not fulfill the requirements of any federal health coverage mandate. Patients are encouraged to obtain and maintain healthcare insurance. A DPC agreement does not include hospital services, emergency room treatment, or any services not personally provided by this practice or its staff. The practice does not participate in any health plans, HMO panels, and/or Medicare coverage.
